This short online form is the quickest and simplest way for the majority of our patients to access our team for assistance with administrative matters and non-urgent medical requests. Our practice phone lines remain open for anyone with accessibility needs who might find difficulty completing this form.

Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during office hours. The forms will not be read on weekends (Saturdays and Sundays) or on bank holidays. On a weekday (Monday – Friday) forms will not be read before 07:00 or after 17:00, We will respond within 4 hours during opening times. We request that the majority of the time, this method is used to contact us. Friends and family can complete the form if needed.

For admin and routine care requests (e.g. booking cervical screening appointments or vaccinations), the online digital contact form, reception face to face and contact via phone are available Monday to Friday 08.00 to 18.30.

Non-urgent advice: When we are closed

When the surgery is closed there is always someone available to look after you if you are unwell. 

Outside of our opening hours you should ring 111 if you need urgent medical help or advice. 111 is a free service available 24 hours a day, 365 days a year.

In a genuine emergency you should call 999. Chest pains and / or shortness of breath constitute an emergency.
